How to Prepare for Your Immigration Medical Exam

What to Bring

Government-issued photo ID — Passport or driver’s license. Every patient needs at least one form of ID, regardless of age.

Vaccination records — Bring any immunization records you have available.

You do NOT need to bring blank I-693 forms — We have all required forms in our office.

No fasting required — You can eat and drink normally before your exam.
