You need to bring the following items to your initial appointment:
- Passport or Driver’s License (Every person needs at least 1 form of ID regardless of age)
- Your available Immunization Records
- You do not need to bring your own blank I-693 forms as we have them available for you.
- You do NOT need to fast for this exam!
