If you are submitting form N-400 for Naturalization, you may be eligible for Form N-648. If you have a true medical condition that impairs your ability to learn new knowledge about US History, Civics, or English, then you may be eligible for the N-648 exam. Before your appointment for your N-648 exam, please prepare a list of medical conditions, medications currently being taken, the name and number of your current primary care doctor, and bring your permanent residence card/ photo ID.
